Since 1982, LCHPS has actively worked alongside community partners to promote, provide and advocate for accessible and quality health, wellness and social services.

 

After consultation and community engagement, Lake Country Health is proud to be working towards bringing a Community Health Centre (CHC) to Lake Country. Our goal is to open as soon as possible, and with your help, we can reach our vision sooner!

A community health centre is a hub for health and social services, to support the needs of the specific community where it is located. A CHC offers primary care, like physicians and nurses, as well as other services based on the needs of the local community.

 

CHC's differ from other clinic models in that they are structured to provide comprehensive and integrated services, and to meet multiple needs of target populations.

What will Lake Country's CHC look like?

What will ours look like?

The Lake Country CHC model was developed after several years of community consultations. It will be a 5,000-5,500 sq. ft clinic that will offer primary care services delivered by nurse practitioners, family physicians, and nurses.

 

Allied health services will also be offered including dietitian appointments, acute physiotherapy, as well as individual and family mental health appointments with a clinical social worker.

 

The CHC will incorporate and expand the current social services and programs that Lake Country Health offers within the community. This includes mental health groups and workshops, a caregiver support program, harm reduction supplies, and seniors services. LCH also looks forward to expanding the programs and workshop offerings, as offerings are currently limited due to a lack of program space.
